The Children with Disabilities Team

The Children with Disabilities Team is a statutory service who work specifically with children and young people, who are aged 0 to 18, with significant special needs and/or disabilities, as defined in the Children Act 1989:

• their disability means they have significantly greater difficulty with physical, learning, social, emotional or educational development than other children of the same age
   
• they have a permanent and significant learning or physical disability sufficient to prevent the child or young person from taking part in activities which are within the capacity of children of the same age
   
• their learning and physical disability issues will result in long-term impairment of health and social development
   
• they have very complex health needs or where a child has a life-limiting illness, and coordination is needed across services to ensure the best possible outcomes
   
• they require constant supervision and are dependent on others for basic day-to-day care needs
   
• they have a significant hearing impairment or visual impairment

We are a multidisciplinary team and focus on working with disabled children, young people and their families to ensure they are safe and there is appropriate support in place to meet their and their carers' needs. We want to ensure children and young people have the opportunity meet their potential, be as independent as possible and have a positive family life.

Children and young people will have either a Child in Need or Child Protection Plan, which are regularly reviewed, to ensure that their needs and carers' needs are being met.

We also provide support for Occupational Therapy and Direct Payments.
