The 16 plus and Care Leavers Team work with young people aged between 16 and 21 years (in some cases up to 24 years depending on a needs assessment).

When you are 16 to 17 years old, social workers put arrangements in place to help you prepare for leaving care when you are 18. They will listen to what you want for the future and help you each step of the way - this can be practically, financially and emotionally. At 18, you will then be allocated a Personal Adviser, who will support you to develop skills to become an independent adult who is in control of your own life..
