West Berkshire Council Reablement Service
Reablement Services help people recover, regain independence and remain at home.
The Reablement Service provides support for a short time to help individuals recover and increase independence, or to review ongoing care needs in their own homes.
Support is provided by a team of people. They will discuss and identify realistic aims for you to work towards during your time with the service.
Reablement may:
- help you remain at home when things become more difficult
- help you avoid hospital admission
- help you after an injury, an acute illness or an operation
- help you return home more quickly after a stay in hospital
Our goal is for people to regain their independence so that they can stay independent in their own home with the minimum level of support. We will set and frequently review goals to help show the progress people have made in regaining their independence.
The service does not accept referrals from the public.
The West Berkshire Reablement service is regulated by the Care Quality Commission (CQC).
